add termination option of "when balance is 0" for markers/special notes

It would be nice if you could tie the termination date to the balance--so if the note/marker has to do with a certain date of service or balance, it will terminate once that service/balance is paid

Have the ability to remove the signature lines at the bottom of the treatment plan print out, we do not have patients sign and when we email them the treatment plan, it is confusing and they think they need to sign and return it, we don't ask that. It should be optional to have that on there.
