There are so many ways you can set up insurance forms, which is create. However, it is also really hard to find out what is wrong when there are so many options.
Would be good if the addresses for the billing and servicing entities were visible, so that you can verify the information is correct or link to edit it
Submittor ID override should not be available if that feature is not in use (grey it out)
The servicing location should NOT be based on where you are logged into when creating the claim. It should remain defaulted to the location the charges were posted at (huge issue for larger groups).
