There is no point providing a form that is not numerically identical to the truform document .ie. On truform # 11 is Rheumatic fever but when you construct your list in OMSVISION it may be a completely different numerical number . Also you have to populate all the info from truform not just a yes answer . A NO answer in medicine may also be critically important
